Immunizations for child 1 not on department approved form. Additionally it appears the child is not up to date with her vaccinations. Check with parent to get the most recent copy of the immunizations. If the immunizations are not up to date you need a signed statement from the parent outlining when the child's vaccinations will be brought up to date.
